Financial : Form for Expense Reimbursement

Instructions for completing the form are below.

Reimbursement Form Instructions

The form must have receipts attached.
The form must be signed by the recipient.
The form must be signed by the committee chair.
The Committee Chair may approve their own reimbursement requests.
Completed forms should be placed in the Finance Committee box.
Properly completed forms will be take about 10 days to process.
